Current Patients: If you need to request a new link, go to the Patient Portal, click "I'm an Existing Client" > enter your email address > click Continue, and you will immediately receive an email with a one-time use link that brings you directly into the Patient Portal. As a security measure, links expire after 24-hours, but you can easily request a new link at any time. Please feel free to contact us if you have any questions. You can also use your patient portal to communicate with your clinician via secure messaging.
New Patients: Our practice has a secure, HIPAA-compliant online portal where new patient forms are completed electronically. If you scheduled a new patient appointment, you should have received an email from our practice management system, with a link to access your patient portal. If you have not yet received this portal link, please check your SPAM folder. You can also go the Patient Portal and enter in your email address to be sent a new link. If you still have difficulty, contact us to confirm your email address. Once you are an established patient, you can also use your patient portal to communicate with your clinician via secure messaging.
Alternatively, you may print the new patient forms from below, complete them by hand, and scan/upload them to your patient portal, or bring them to your initial intake appointment. Forms completed by hand can also be faxed to us at (561) 705-0895, if preferred. * Please complete your new patient forms at least 48-hours prior to your initial appointment, to allow ample time for us to review your information.
